obtaining it.
1) Provide your demographic information
2) Provide your insurance information to verify coverage
3) Obtain a doctor’s order
Note: Information can be filled out on the Intake Form which is located at the top right corner of this website.
Therapy can be scheduled about a week after we receive the doctor’s
order. If a prior authorization is required, then 2-3 weeks is needed.
